Qualifications
- High School Diploma and experience working in social services required.
- Bachelor's Degree and experience working in the field of social services with individuals with disabilities is preferred.
- First Aid and CPR preferred.
- Driver's license and current insurance required.
Primary Duties/Responsibilities
- Assists participants with meeting employer expectations with work performance.
- Provide training to improve participant's work performance after employer's training period has ended.
- Problem solves to create accommodations or other ways to address performance issues.
- Support participants to develop and maintain natural support and positive relationships with co-workers and supervisors.
- Completes and submits agency documentation in a timely manner such as weekly progress notes and hourly billing.
- Maintains participant file in accordance with agency requirements.
- Utilizes time at participant's the work site to complete necessary documentation, when possible, maintains a minimum of 80% of hours worked in billable time.
- Maintains knowledge of community resources that can be utilized to support participant needs (i.e., transportation, medical, childcare, utilities, etc.) Makes referrals, advocates, and follows up.
- Maintains confidentiality according to agency guidelines, follows ethical guidelines.
- Completes training hours annually based on funded requirements.
- Other duties as assigned.
