About the Role

As a Crisis Case Manager, you will play a vital role in providing support and assistance to individuals experiencing crises and navigating through challenging situations. You will work closely with clients, families, and other professionals to develop and implement effective crisis intervention plans, ensuring the safety and well-being of those in need. This role requires strong problem-solving skills, empathy, and the ability to work effectively under pressure.

Primary Duties/Responsibilities

  • Conduct thorough assessments of clients' crisis situations, including mental health status, safety risks, and immediate needs. Intake can take place in the community, court or at a participants’ residence.
  • Develop and implement crisis intervention plans, providing appropriate resources and referrals to support clients in crisis.
  • Collaborate with clients, families, and other professionals to create comprehensive crisis management plans tailored to individual needs.
  • Provide emotional support to clients, utilizing active listening and empathy to establish rapport and build trust.
  • Monitor and evaluate clients' progress throughout the crisis intervention process, adjusting plans and services as necessary.
  • By means of a formal referral to the court this position will connect and coordinate participants with community resources, such as mental health agencies, housing assistance, healthcare services, and financial support through means of
  • Advocate for clients' rights and access to services, ensuring their needs are met and their voices are heard within the community.
  • Maintain detailed and accurate case documentation, including assessments, progress notes to be used to generate a monthly report to the court by the 10th of the month.
  • Collaborate with interdisciplinary teams and participate in case conferences, sharing insights and recommendations for participants' well-being.

  • Ability to work effectively as part of a team, collaborating with other professionals, agencies, and community resources to meet clients' needs.
  • Strong organizational skills, with the ability to prioritize tasks and manage multiple cases simultaneously.
  • Familiarity with community resources and services available for individuals in crisis.
  •  Proficient computer skills, including experience with case management software and electronic documentation systems.

Qualifications

  • Bachelor's degree in social work, psychology, counseling, or a related field preferred.
  • Previous experience in crisis intervention, case management, or a related field is highly desirable.
  • Knowledge of relevant laws, regulations, and ethical standards related to crisis intervention and client advocacy.
  • Ability to maintain confidentiality and adhere to professional boundaries.
  • Valid driver's license and access to reliable transportation for home visits or community outreach.
  • Ability to pass a background check and any additional requirements in accordance with local regulations.
